HCHD RESEARCH PROTOCOL

Instructions for conducting research at Lyndon B. Johnson General Hospital, Thomas Street Clinic, or Community Health Clinics:
 

1.Prior to conducting any research in the aforementioned facilities, approval from the UT Committee for the Protection of
Human Subjects (UT CPHS) and the Harris County Hospital District (HCHD) must be obtained.

2.Approval by the UT Committee for the Protection of Human Subjects is a pre-requisite for HCHD approval.

3.Following UT CPHS approval, a separate research application must be sent to the Harris County Hospital District (HCHD).
The HCHD application packet and research policies are below:

Please note that you must submit one copy of the completed HCHD research application packet to LBJ Chief of Staff’s
Office(Attn: Dr. Bungo).
