COMPUTER WORK REQUEST

Please follow the instruction below regarding your computer work request.

• Click on the link http://med.uth.tmc.edu/msit/mshelp.htm to open the form and log in with your username and password.
• Select New Ticket to open your ticket and make sure you click on Choose Dept and select LBJ for department.
• Fill out the Request and click Submit Request.

The information provided will assist us in determining a solution to the problems you are having. Once your work order has been completed it will automatically be sent to your network specialists. Your request will be handled on a first come and first serve basis.

When the network is down, please submit a paper Work Request Form. These are available in the Chief of Staff Office.
